FORMS FOR IGNOAPS, GNDPS, IGNWPS, NFBS

Shillong, January 13, 2011: The Deputy Commissioner, East Khasi Hills District has informed that the application form for IGNOAPS, GNDPS, IGNWPS and NFBS are available in the Deputy Commissioner’s Office, Room No. 314 during office hours. Forms will be distributed on production of documents as indicated.

For IGNOAPS (above 65 years of age), documents needed are BPL Card, EPIC, Headman Certificate, for GNDPS (18 to 64 years), disabled certificate from Medical Board, East Khasi Hills, Shillong, EPIC, BPL Card. For IGNWPS, BPL Card, Death Certificate of husband and Headman Certificate and for IGNFBS (18 to 64 years), documents needed are EPIC, BPL Card, Headman Certificate, Death Certificate of the bread winner (male only).
